Evaluation: Yout mother should be evaluated. The symptoms are suggestive of dehydration but the underlying cause needs to be determined.

Needs...: She needs to see someone as soon as possible. I don't have the information of her age, usual diseases she might be suffering of, etc. So if she feels this sick I think it is time to go to the ED for evaluation.
