You really need to: coordinate this with your oncology team. They have a better grasp of your relatively complicated medical history, your other meds, what is safe to prescribe, and the dosing. We would really be overstepping boundaries if we were to start pain management exclusively via telemedecine. They know how your liver and kidneys are doing, and how all the drugs need to be juggled.
