It is hard to tell: from what you are describing. Are these issues new ones or have you had similar problems earlier in your life? How old are you? Have there been any serious stresses lately (family problems, deaths of loved ones or financial challenges). 1st speak to your PCP to rule out medical causes, if they rule out physical issues then consider psychotherapy.
