Adrenal testing: The pituitary adrenal relationship has a diurnal rhythm. Plasma cortisol peaks in AM around or just after sunrise, then dips to lower values in late PM, after sunset. ACTH infusion testing can be done at any hour. Interpretation must include pre-ACTH plasma cortisol value and post-ACTH rise. The post-ACTH rise should be more than 50% and optimally a two-fold rise. Time of day matters little
