Abscess: One may have an abscess... when lying down, the blood pressure in the head increases, often causing a throbbing sensation on the affected tooth.

Blood circulation: When one lies down, the blood circulates to the neck and the head area. That's why toothache is more likely to get worse during night when you lie down.
