Alters GI flora: Likely because doxycycline alters gastrointestinal Flora and can cause Clostridium difficile infection and elders can be more at risk

Hard to say: Most commonly it would be related to either a drug drug interaction or an adverse reaction to doxycycline in the past. Otherwise it's actually a reasonably well tolerated medication.
