Many things. Poor nutrition, continued trauma and/or stress to the wound, poor arterial blood supply to or poor vein blood return from the ulcer, the wounds has excessive bacteria (bioburden) that prevent healing and more. A true wound care specialist can identify the causes and get them resolved and the wound healed.
