A member asked:

Does the depo-provera shot only fail if it's not taken every 12 weeks?

A doctor has provided 1 answer

Any method can fail!: You are most likely to be protected if you get the DepoProvera injection every 3 months. But nothing is failsafe! Many couples use barrier methods like condoms in addition if they are worried. Naturally, if you go beyond the 12 weeks and get your injections irregularly, your risk of pregnancy increases.
