Not much: Symptomatic external hemorrhoids are large and very painful, occasional bleeding; the pain will be so intense that you may have to see a surgeon or go to the ER. If it doesn't hurt much avoid constipation by eating more fiber and drinking plenty of water, do warm sits bath 3 times a day follow by hemorrhoid cream after each bath and after each BM. Avoid hot spicy foods too.
