Musculoskeletal: This sounds more like a musculoskeletal discomfort related to the shoulder or upper chest wall(?strain) and not connected to the reflux. Motions of the shoulder would affect the discomfort or there may be tenderness of the rib cage which would point to the diagnosis. Stress and tension could aggravate the symptom.
