My nephew had a rash as my sister told me but when i changed his diaper i noticed the rash is blisters an now around parts of his hips an ankles?

Please have him: seen by his doctor. It is hard to diagnose a rash without seeing it. This could be a reaction to something, but also could be an infection, either viral or bacterial. You didn't mention your nephew's age; if he is an infant, I would be more concerned and call his doctor today.
