Possibly: 1) He may well have ejaculated pre-cum which he did not sense. This is secretion from Cowper's gland and often contains living sperm. Hence "pull-out" is totally unreliable for contraception. 2) He may well have some semen remaining in his urethra after wiping his penis. This residual semen will exit his penis after he reinserts. Thus, sorry to say "sperm may have entered". Use condom to be safe.

Probably: there was likely still some semen/sperm on the penis and certainly possible for more ejaculate to be released again. so most likely there was some sperm that made it inside

Yes: if you had unprotected sex, you can get pregnant. Coitus interruptus or pulling out before ejaculation is a very poor method of contraception and can get you pregnant despite your best efforts. There are various forms of contraception available, please speak to your doctor to choose one that works best for you.
