Your choice: When getting a pelvic exam it is fine to do whatever makes you comfortable. Talking to the nurse or doctor is fine if that helps distract you from the awkwardness. I have to wonder however why you are getting pap smears at all at 16 years old. They are not even recommended until you are 21 years old. Also just for your information, there should always be a chaperone in the room with the doctor.

First time?: if this is your first pap smear, make sure first to choose a female health care provider, if that will make you more comfrontable, if you could not choose, make sure you tell the nurse she is your first time, and the nurse usually keep you busy or occupy, and she or the doctor will be talking to you telling you what to expect, relax it is not as bad as it sound.

PAP: PAP smears are not recommended till age 21 (even if sexually active)because pathologies resolve spontaneously in the young population, doing a PAP would lead to unnecessary procedures. Start by an appointment to discuss they type of screening needed at your age and if you have risk factors like DES baby. If a PAP is needed, there is a nurse in the room so you can talk to her about women's health.
