It depends: On where and how big and for how long. The part that is not getting enough blood (that's why you get a heart attack) will not be moving as much, and because of inflammation and not getting blood, that part can look anywhere from pale to kind of purple and angry looking. If you are having emergency surgery because you are having a heart attack, that's what happens, and that's why you can see it.
