An evaluation: with serial exams by a Behavioral Neurologist is in order. There are two at Mayo Jacksonville and two at the University of Florida (there is a 5th in St Pete) It would be helpful to know what type of MCI you have (single domain, amnestic, multi domain, etc) also it is important to know about treatable / reversible conditions like vitamin deficiencies (B12 /D3) and other comorbidities.

Brain atrophy: Brain atrophy means shrinking of the brain caused by loss of neurons. It is a normal finding in aging although this does not lead to equivalent cognitive changes. It also accompanies many neurodegenerative diseases most of which do not have a reversible quality. Causes that can be treated include B12 deficiency, alcohol use, inflammatory and infectious processes. Your doctor should be telling you why you have this and identify measures that can help maintain function and minimize atrophy to the degree possible.

Some highlights: It is very likely that you have atrophy dating back several yrs, and the first question involves a hereditary frontal dementia, which will have a specific pattern on an MRI. Think a neurologist could help sort out the precise diagnosis, as could lead to more effective interventions.
