More than 1 thing: Hi. There is no unifying underlying condition. I suspect the DM and HTN caused the CKD. Assuming you're type 2, the psoriasis is an independent and unrelated disease. Are you conscious with the jerking? If not, it could be a seizure. If so, it could be hypoglycemia from too much insulin (altho can lose consciousness). Falls in BP can be serious, but also, BP does vary throughout day.
