Diet first: The diet is one of the most important treatments in GERD-avoiding trigger foods, such as coffee, alcohol, spicy foods, fatty foods etc, all of which can increase acid production. Follow-up with the Dr. and see if you need any further testing. After testing, if found to be certain hyper secretory conditions, you may need the medications twice a day for a period of time. Best wishes

Sounds: Like you need a scope. If Nexium (esomeprazole) is not helping and you have a bitter taste in your mouth - that is likely stomach acid. Switching to a different medicine is an alternative but sometimes you may need an endoscope to ensure nothing else is contributing to this finding. See your doctor and good luck.

Review this with: Your doc. It's possible that this is nonacidic reflux related as 40 mg daily provides a reasonable degree of acid suppression. In the meanwhile, you should try to remain upright 1-2 hours after eating and consider sleeping with either several pillows under head or with the head of your bed propped up 6 inches or so. Best of luck.
