Many things: If his other milestones are normal then this narrows it down a bit. We want to know details of his birth history, early life, nutrition factors, overall growth, and family history to just name a few things in order to figure this out. Is the motor skill delay progressive? (is his muscle strength WORSENING?) Or is it static? Could be many reasons at this point, these answers will guide diagnosis
