Constipation: The possibility exists that he is constipated and needs help to have bowel movements come. However, with this extent of abdominal pain and it lasting since weds, I suggest you take him to an urgent care or er for evaluation.

Lot of possibilities: Today is Sunday so this sounds like a four day history of abdominal pain? The mention of " rolling around in pain" is concerning to me. The "very severe" nature is also concerning. It may be something as simple as constipation, but I'd like him to have a more thorough history and exam very soon by his physician. His exam will really point us in the right direction.
