More likely CAH: Hi. The most likely cause of elevated androstenedione (A-dione) is congenital adrenal hyperplasia (CAH), a condition in which an adrenal enzyme involved in steroidogenesis is sluggish, and side pathways producing adrenal androgens (A-dione, DHEA, DHEA-S) become prominent. CAH usually causes hirsutism and irregular periods in women; severe cases salt wasting. See an endocrinologist for evaluation.

Don't know what a: "moderate" elevation or a "slight" elevation is, but PCOS is the most likely. Discuss potential use of metformin with your doctor, and even adding in spironolactone to lower testosterone.
