See a pediatrician .: You should bring your 20yr baby to see a pediatrician for general health check for nutrition, growth, and development, and specific growth of legs, etc. so she/he may sort out what may cause such - inborn or acquired - and recommend how to manage the current concern about baby's legs. So, act ASAP.

Depends: If there is a similar degree on both sides that mirror each other these can self correct. These are monitored through their regular checkups for progression in the right direction. I can't recall any needing surgery in more than 3 decades in practice & few needed any PT.
