Site reaction: Injection site can signal a negative reaction to the drug. Or amount injected. Rarely, a nerve could be injured that develops a neuroma, and remains painful. Infections can also occur. Typically this should resolve in a few days, but if it doesn't , then call your provider and get examined to determine if this is serious.

Hit a nerve: Hi. Almost certainly the needle hit a nerve, causing it to fire and send a pain signal. That happens sometimes. Fortunately, usually it passes fairly quickly. Better luck with the next on! :-)

Unsure.: The Depo shot is given in a moderately large muscle like the deltoid. Severe pain suggests that the injection technique was incorrect. The arm bone or a tendon may have been inadvertently hit or the medication pushed in too quickly. Apply ice to the area several times a day and take OTC pain meds for immediate relief. Tell your doctor about this before your next Depo is administered.
