See Your Doctor: Your description suggests you may have an anatomic anomaly possibly a urethral pit, a dimple which may/may-not be connected to internal structures. Can be seen w/ Hypospadias, an abnormal location of urethral opening usually on underside of penile head. Examination of your genitalia is necessary to make any conclusions. See your doctor for proper diagnosis & treatment.

Hypospadias: Probably hypospadias if you urinate thru the tiny hole or so called fistula. Much less likely opening of a second urethral opening if you have a double urethra. Advise urology consultation if urine comes out of it.
