Sciatica pain: Usually need impinging thecal sac or nerve roots and weakness/loss of sensation for bulge to be significant. May be piriformis syndrome- after sciatic nerve exits the spine, compressed by buttock muscles( talk with PT). Consider intermittant baths with 1/4 cup Epsom salt MgSO4),, check magnesium, Vit D , gait ,pelvic tilt,core,try kinesiotape,decompression. F/u with physiatrist- PMR M

SIJ: 27 with chronic sciatica has insignificant imaging pathology of spine & lacks benefits from P.T. Sciatica is a pain pattern which can be aroused by a variety of pathologies. A common cause of chronic sciatica is sacroiliac joint disorder. Female of this age might have Joint Hypermobility Disorder with associated sciatica. Pyriformis Syndrome is another possibility.
