Not normally: Because straining increases blood pressure, it is possible to have a stroke this way. This is rare though. Most strokes are not caused by a sudden rise in blood pressure, but by changes of the arteries over years that result in fragile debris on the inside of the artery breaking off and lodging in smaller artery in the brain.
