Are antibiotics a must if sinusitis lasts more than ten days? i'm allergic to many and have a c diff history. c diff free for year but nervous of atx.

No: You evaluation will clarify if you need systemic antibiotics. You need cultures and imaging to try to be as specific as possible if antibiotics are ever a need you you for anything.

Not a must: I would not say they are a must. Our guidelines recommend treatment for sinusitis for symptoms of a cold persisting between 10 to 14 days with no improvement. That said, I prefer waiting the 14 days in patients who are not febrile, without facial pain and erythema. I would take this approach especially in a patient with multiple allergies and a c diff history. Feel better!
