A member asked:

Is there ever a time where prolonged swelling of lower leg and no definitive diagnosis requires making an incision to find the cause of the swelling?

Swelling: Evaluation to determine the source of the swelling, may include sonogram, CT scan of the abdomen and pelvis. I can think of a reason why an incision would be necessary to determine the source, unless there is a suspicion of a tumor causing the swelling. I would recommend use of a compression stocking during daily activities to treat the swelling issue.

No: No. With today's noninvasive tests available such as MRI, ct scan, ultrasound, venogram, etc; there would be no real indication just to go in and "look" into a swollen leg without having a pretty good idea about the cause first.

No: Quite honestly, making incisions are likely to make matters worse because of permanent disruption of the lymphatic channels in the leg. Elevation, compression socks, and massage are your mainstays of treatment. Surgery doesn't cure everything, and in some cases, can make matters worse.
