Hydronephrosis: Swelling of the kidney due to backing up of urine caused by obstruction to its outflow can cause infection with shaking chills and fever and can cause back/flank pain. Sometimes it's silent,however.

Flank pain, hematuria or no symptoms depending upon cause. Obstructing kidney stone usually causes severe pain, flank tenderness & microscopic hematuria. Associated with infection can cause chills and rigors. Hyronephrosis from kidney outflow obstruction can cause "Dietl's crisis", episodic pain + possible nausea & vomiting related to sudden fluid load to kidney after after drinking.
