Nothing: GFR obtained from creatinine levels is a crude and frankly pseudoscientific estimate that's only useful for kidney failure patients. I'm going to assume that creatinine of 74 is in the reference range for the lab and the units. The GFR of 81 ml/min almost certainly is. Don't pay too much attention to your labs unless they are very abnorma or you're anemic. Best wishes.
