Needs exam: Please take your friend to a physician for a full history and physical exam including a neurological exam. He may need imaging of the brain or spine. He will probably be tested for B12 level and other blood tests. Muscular weakness, balance problems, and back pain are worrisome indeed. If no physcian, may take him to the ER.

Yikes!: I tend to think worst case scenario - better to be wrong than miss something. For an otherwise healthy teenage boy to develop limb weakness, falling, and back pain, one has to rule out a spinal cord lesion. He needs to get imaging immediately, including spinal column x-ray series plus brain & cord MRI with & without contrast. May need spinal tap with cytology too. Get him to an ER STAT.
