My wife has a swelling almost like a very large pimple right next to her hip bone. at one point a lot of puss came out but it's still swollen. worried?

Infection: Sounds like it's infected. Sometimes when the puss comes out, the infection will clear up, but since it's swollen there likely is more puss in there and could be infected. Get checked out to see if she needs an antibiotic.
