Many different kinds: but probably most people would start with a psychiatrist with experience in addictions. There is a specialty of addiction psychiatry (www.aaap.org) and addiction medicine (www.asam.org). Your family physician would be another place to start, as many internists or generalist physicians develop expertise in helping you with these problems, as well as referring you to programs with more expertise.
