Trying to conceive: your PCP will probably refer you to an OB-GYN or someone who specializes in fertility problems.

Straight-forward: Just tell the receptionist that you need an appointment to discuss infertility with the doctor. Once you see your PCP he/she will likely refer you to a fertility specialist. Usually internists do not treat infertility.
