Not likely: If there was already a pregnancy, then you would not ovulate, so no, you should not get pregnant from the insemination.

No: You don't become fertile again immediately after a miscarriage so the insemination 3 days prior to a miscarriage couldn't get you pregnant a second time. Best wishes!
