Enlarged spleen: left upper back side pain maybe an indication of enlarged spleen, maybe due to viral illness such as mono, or possibly related to muscle injury or spasm. You also taking blood thinners, so yo are prone to splenic hemorrhage, you may need an ultrasound or ct scan to evaluate spleen

Ultrasound + Bloods: Abdominal pain can be caused by many different pathologies. If this is linked to breathing, then there are chances it could be a rib cartilage strain. However there are other structures nearby where the pain could originate from such as the spleen, ovaries, fallopian tubes, kidneys, and ribs.If pain is ongoing you will need to see a Dr and get an ultrasound and blood tests done.
