Variable: Anal fissures can be difficult to heal. It may take 2 weeks, it could take 2 months or more. The potential problem is that the fissure can re-tear after bowel movements. That is why you need to drink plenty of water daily and take fiber to keep the sttols formed but soft. Do not strain. If the fissure does not heal with conservative therapy (and many do) then surgery is needed.

Depends: Without surgery and with adequate treatment not all chronic fissures will heal. If it does not heal in 8 weeks with treatment you may need to resort to surgery. The sentinel pile will not go away without surgery.
