See surg.oncologist: Lymph nodes the size of a golf ball are usually neoplastic and not inflammatory. Being firm and fixed adds to dx of either primary or metastatic tumor. Most metastatic lesions of the H&N have an easily defined primary of oral cavity or thyroid. Primary lesions here are usually lymphomas. After H&N exam an excisional bx and not aspiration should be performed.
