Needs another visit: Six days of a high fever, even in the absence of physical findings, deserves at least a urine and blood test as well as possibly a chest xray. Urinary tract infections are often missed in febrile toddlers. Pneumonia can be hard to hear on an exam. All of these could be due to the virulent enterovirus now circulating across the US but one cannot be sure or exclude other sources without testing
