What would cause tension in left shoulder blade, muscle spasms in shoulder blade, chest, and in upper outside of arm (not bicep)?

Heart conditions can cause left upper extremity symptoms. Also, a pinched nerve in the neck can cause the symptoms you describe. If your symptoms are persistent/severe then you should definitely see a physician esp to evaluate for possible heart problems.
