Rectal urges: What you describe could be from many things including severe constipation (a fecal impaction), but one of the more common causes of that type of urge and what we call tenesmus is from proctitis ( a mild form of colitis involving only the rectum). The cause is not known but it is treatable with topical medication placed in the rectum at night
