If you really had: a total thyroidectomy, you need thyroid replacement life long, however, 200 MICRO grams a day would be a relatively high dose. If your TSH is 4.6 after no replacement, your surgeon left in some thyroid tissue, but you still need BRAND NAME thyroxine replacement at an appropriate dose to keep your levels normal.
