The main reason: The 100 patient limit. Also many doctors are doing this solely for the dollar signs and the reasonable one are at their treatment limits

TRAINING & INTEREST: There is a training program that is involved. Additionally each doctor can only have a maximum of 100 patients,. Also a lot of monitoring is needed ie drug testing and prior authorizations. Most importantly the doctor has to have the interest and the patience in this area of substance abuse.
