A few days: The pain should subside over 24-72 hours. Congestion and ear fullness can take a little longer. This is assumes that the bacteria is not resistant to azithromycin. If pain, fever, headache jaw pain or dizziness develop/worsen, you should be re-evaluated for resistant infection and treated with a broader spectrum antibiotic.
