Several days: It will take several days to clear the infection . Don't forget to use nasal saline washes and nasal decongestants to help open up the sinuses and Eustachian tubes.

A few days: The pain should subside over 24-72 hours. Congestion and ear fullness can take a little longer. This is assumes that the bacteria is not resistant to azithromycin. If pain, fever, headache jaw pain or dizziness develop/worsen, you should be re-evaluated for resistant infection and treated with a broader spectrum antibiotic.

Varies: Azithromycin is a sticky drug that builds up in the system and stays there for DAYS after the treatment stops. 30% of the time it fails in common strep infections. 100% of the time it fails in common VIRAL infections. cannot say in your particular case.
