A clinical diagnosis: is one made by history and exam criteria, rather than by lab or X ray results. A good example is clinical depression. There is a depression screen that can be performed which identifies the disease when no blood tests are available. Hope this helps!

It means: Making a diagnosis based on the history a patient provides, along with an examination of the patient, without the additional information from lab testing or imaging(like x-rays).
