Need urgent eval: Vomiting coffee grains could be a sign of blood loss. Ideally it would be nice to have a scope to determine whether you are actively bleeding. The safest bet is to go to the emergency room with this severe pain to get a better history and work up. Good luck

Be seen: Dark, coffee ground appearing vomit could signify bleeding in the stomach or upper GI tract. Common causes are an ulcer or gastritis, (an inflammation of the stomach lining). I see you had an allergic reaction to a bug bite, if you took steroids for your reaction they could predispose you to this. I suggest being seen as soon as possible.

Coffee ground emesis: I'm not sure what the antibiotics are for, but what looks like coffee grounds can come from digested blood. You are "off your food" b/c ur body is trying to tell u something is wrong. Stomach ulcers, esophageal tears, or hemorrhagic pancreatitis could be the cause (do you drink alcohol?). Get to an urgent care or hospital ER so that a GI doctor can arrange for and endoscopy to make a diagnosis.
