What state's are allowed to consult over the phone or video and order any test if physical evaluation is not necessary?

Not for sure: I would recommend you call the state medical association and they could give you the information for that state. I would recommend going to the Association of American Physician and Surgeons. They would likely have information to help you with your need. They have a site called Medi-Bid and it links patients and prroviders together for tests, xrays etc. I hope this helps.
