Check several places: Often these are present in baby books, on school entry records, or from your doc. In my state, I am required to keep records until the kid turns 21 or 7 years from the last visit, whichever is later. They are then disposed of by a certified center. Blood tests can reveal antibody levels that would show if you had the disease or were immunized.
