Get checked: This one is for the doctor who casted you. You could have a compartment syndrome which is an emergency. You could have delayed healing of your sprain, granted your ankle/foot was not broken. Please see your doctor as soon as possible or to the emergency room

See your doctor ASAP: You suffered a severe sprain of your ankle with complete tear of ligaments (likely the ones on the outside of your ankle). If all went well you shouldn't be having "tremendous" pain with immobilization. You had best call your doctor immediately and tell him/her about your symptoms.
