See his physician: Sometimes, even without trauma, one can rupture a tendon connected to part of the biceps muscle. He needs to see his physician. Usually the treatment involves rest, pain medicines and physical therapy. If he is a serious athlete, he should see an orthopedist for an evaluation

Needs checked: He could have torn, sprained his arm, developed bursitis and worse case scenario developed a blood clot. I would recommend an urgent exam by his doctor.
