Seek help now: If your wife is not already under the care of a nephrologist, I highly recommend seeing one ASAP. Is there any nausea, unexplained bleeding, inability to urinate with or without a full bladder? Diarrhea or vomiting? By the numbers alone, she has a serious kidney issue that needs to be addressed sooner rather than later. Whether or not she has symptoms, she should seek help immediately.
